Application for a U.S. Passport

The standard form for first-time applicants, minors under 16, and anyone whose last passport was issued more than 15 years ago, or was lost, stolen, or damaged.

U.S. Passport Renewal by Mail

Allows eligible adults to renew their passport by mail — no in-person appointment needed. Your most recent passport must be less than 15 years old and issued after age 16.

Statement Regarding a Lost or Stolen Passport

Must be submitted before applying for a replacement passport. Filing this form officially invalidates the missing passport and protects you from identity theft.

Statement of Consent for a Minor's Passport

Required when only one parent accompanies a child under 16 to apply. The absent parent must sign this form in front of a notary.

Used to correct errors in a recently issued passport, update a legal name change, or replace a limited-validity passport book — at no charge within one year of issuance.

Substitutes for the second parent's signature on DS-11 when that parent is absent due to documented special circumstances such as military deployment or incarceration.
